1. Acceptance of Terms
By using IntakeIQ, you agree to these Terms. If you do not agree, do not use the platform.
2. Description of Services
IntakeIQ is a HIPAA-compliant SaaS platform for client intake, document collection, and long-term record retention. Use of IntakeIQ does not constitute a Business Associate Agreement (BAA) unless explicitly signed by both parties. You are responsible for executing a BAA with us if required under HIPAA.
3. Account Registration and Security
You are responsible for your credentials and all account activity. Notify us of any unauthorized access.
4. Privacy Policy
Use is governed by our Privacy Policy, which explains how we process and protect your data.
5. User Conduct
You may not:
- Violate any law
- Share PHI without proper consent
- Abuse affiliate or referral programs
- Interfere with other users or system security
6. Data Security
We use HIPAA-aligned protections: encryption, logging, and access control. No system is 100% secure. Use IntakeIQ at your own risk.
7. Subscription and Billing
- Subscriptions renew monthly or annually
- Intake credits are granted and expire per our Credit Policy
- All fees are non-refundable
- Disputes or chargebacks may suspend access
8. Cancellation & Dormant Access
- You retain access until the end of your billing period
- Dormant access (read-only) is available for $99/year
- Data will be deleted after 30 days if no retention plan is selected
9. Termination
Accounts may be suspended for violations, including abuse of affiliate/referral programs.
10. Limitation of Liability
Maximum liability is limited to the amount paid in the previous 12 months.
11. Governing Law
Florida law governs these Terms. All disputes are resolved in Orange County, FL.
12. Changes to Terms
We may update Terms at any time. Continued use implies acceptance.
